How do I change my name by deed poll?
In order to change your name by deed poll you will need to complete the application form online, then send through to the solicitor a copy of your birth certificate, passport or in the case of ladies, then a marriage certificate is also acceptable. You must pay £29.99 which will be processed through the online system.
What documents do I need to provide?
- Copy of passport.
- Copy of birth certificate.
- Copy of marriage certificate (ladies only).
- Parental authorisation if the child is under 18 years.
Do they have to be the originals?
No, please do not send the original documents, a photocopy is perfectly acceptable.

You are legally bound to advise official bodies of your name change as soon as you have the certificate, however there is no deadline date that you have to have this done by.
